Specialist Referral Process
Referral Request Process
If your child needs to see a specialist, please call our office at least 72 hours in advance to allow time for a referral to be completed. Many insurance companies will not accept backdated referrals. Without a timely referral, your insurance may deny the visit or pay the specialist at a lower rate.
Please have your insurance card available when calling our insurance department at (859) 371-7400.
Verify Provider Participation
Be sure to check with your health plan to confirm that our doctors are participating providers in your plan.
Special Referral for Children’s Hospital
If your child is referred to a specialist at Children’s Hospital, a special referral form must be completed by your doctor before scheduling the appointment. Please contact our office to ensure this form has been processed.
Health Plan Claim Forms
We will complete any routine health plan claim forms at no charge. However, additional communication by our staff or doctors may incur a charge.
